Before you start
Have your insurance card ready, including images of the front and back. Choose the service you want reviewed and enter your city and county so we can confirm care and appointment options for your area.
You can ask about prenatal midwifery, postpartum care, planned home birth, doula support, lactation, or classes. Each service is reviewed separately.
What happens next
Our team reviews your plan, the requested service, and any referral or authorization requirements. We’ll reach out if we need more information and explain what to do next.
A benefits review is separate from a coverage decision, an authorization, or a confirmed appointment.
